### What happened

Healio reported on September 25, 2026, that the FDA has approved obinutuzumab (Gazyva, Genentech) for idiopathic nephrotic syndrome. The indication is narrow. It is meant to lower the risk of relapse in people with frequently relapsing or steroid-dependent disease that began in childhood, once they are in remission. Healio frames it as a pediatric approval from age 2, but the FDA wording also covers adults whose disease started in childhood.

### What it means

The approval rests on INSHORE, a phase 3, open-label, randomized trial of 85 patients aged 2 and older. Obinutuzumab was given intravenously on days 1 and 15 and at weeks 24 and 26\. The comparator was twice-daily mycophenolate mofetil. The primary endpoint was a first-morning urine protein-creatinine ratio of 0.2 g/g or less at week 52, with no relapse between weeks 8 and 52\. According to the release, a significantly greater share reached it on obinutuzumab. Common side effects include infections, infusion reactions and neutropenia. The label warns about hepatitis B reactivation and progressive multifocal leukoencephalopathy.

### What we don't know yet

How big the benefit was. The report gives no percentages or confidence intervals, and it is based on a press release. Healio calls it a developing story.
